Servers stuck in "initializing" state
Servers of our "php5" role appear to boot up and be getting stuck in an "initializing" state.
The servers normally enter a "running" state within 5-10 minutes, however the oldest current has been initializing for 40 minutes.
Some, which I have now terminated, had been stuck initializing for hours overnight.
Do you know of what typically causes this condition?

Lots, I am trying to decipher it...
(I have rebooted the two oldest "php5" servers which are running (they had crashed due to heavy load), and a couple in the initializing state (which now are stuck in "running (rebooting)", if that perhaps explains a little of some of what can be seen.)

The "BlockDeviceAttached" messages are not being sent or delivered, however if I login to a server stuck in the "initializing" state the block (a 50GB EBS in the same zone) does appear to have been mounted fine... (see last device mounted in screenshot)

Indeed, scalarizr appears to acknowledge the block mount, but encounters trouble thereafter, which I am not able to understand.
Here is an example of the output from the scalarizr log on a server stuck in the "initializing" state (see screenshot).

Confirmed, there is a problem with Scalr-Scalarizr interaction. We are working on a fix.
